Lifeline Requirements

Valid Photo ID ( Can be driver's license or state-issued ID)

Proof of current benefits ( Oklahoma or Federal-based). Medicaid and Social Security Disability(SSI) are accepted, however, Medicare and retirement Social Security are not acceptable forms of benefit
